"So , if I fall sick you're going to send me to a government set-up? "The physician asked . #COVID #coronaera

Heylo people!

What's on? This lockdown I have decided to make my self more aware of the things I was ignorant about... The areas of Geriatric and pediatrics... I know you guys are honing your culinary skills , but my mind just refuses to go to kitchen. Lets end the banter here.
Today's scenario

We had an elderly female 80 odd years who presented with respiratory symptoms, I was the doctor attending her. She was by protocol isolated , given basic care. My team that comprised of nurse , doctor , housekeeping all wore Personal protective equipment.
The lady was old follow up case of the hospital for decades now. The family was very well off . Ready to do anything to keep her in good health , she was already bedridden. Her Xray Chest was suggestive of Pneumonia. The same day a rule (not on paper) was about to be passed. That all respiratory cases have to be diverted to the government setup. So the decisions with respect to the respiratory cases were taken by the hospital leadership for the day. They suggested that we divert the case.

Meanwhile, the leadership was discussing the plan of care. The treating physician walked in . He was ready to admit the patient in isolation ICU . There was resistance from leadership.

He pointed out something that struck by , " You're sending  old patients away. What if I become a patient and have COVID . You'll send me to government setup? " There was a lot more he spoke about, chopping it  off because of obvious concerns.

Post this , I have a bunch of questions and realisations going on in my head.

1. You die the way you're destined to , whatever be your socioeconomic status.

2. Does the fear of being treated at a government setup create fear among the wealthy ?

3. Is it the major cause of under-reporting ?

4. Is it the cause that some of the physicians have just shut down their Out patient clinics?

5. Is good care and comfort as important as the definite treatment?

6. What's your take on private hospitals treating COVID 19 in their setups?

What are your thoughts? comment below...

So far so much...

MRCEM OSCE preparation and resources

Hello people Since I have cleared my OSCE in one go, I feel it's my duty to journal what all I have done to make it work.(humble brag). Thanks to ChatGPT for helping me pile up some humble generic sentence to make this blog wholesome.(Just kidding). The MRCEM OSCE exam is an integral part of the qualification which tests the clinical and communication skills of candidates. For Indian students pursuing this qualification, preparing for the MRCEM OSCE exam may seem daunting. However, with a few tips that helped me sail through fine. The thing that helped me the most is acclimatizing myself to the UK Clinical Setting Before appearing for the MRCEM OSCE exam, it is imperative for Indian students to understand the clinical setting in the UK. Sometimes , all they want to see is how you behave , your body language, your tone , and your organising skills. Diagnosis is not a critical thing here. You will not fail if you get a wrong diagnosis provided that you have done it systematically. So

" Consulting a psychiatrist would be a question mark on my spirituality " Said the patient.

Hello everybody  I hope everybody is safe and healthy. it's been a while that I've been receiving lot of patients in the emergency with mental illness ,thought of sharing an incident of its kind. Not sure if its the awareness of mental health these days or the pandemic, I am developing keen interest in patients with mental illness .I try to take an extra effort to make them feel comfortable and try to create an insight of what is happening and offer them options of what all can be done.  Since this it seems like a step towards patient care , I thought I'd share it here as well. This would probably not be the best approach but I welcome any type of constructive criticism , additions or subtraction to my approach because end of the day we are here for best patient care. A 50 years plus old gentleman ,presented to the emergency with the complaints of chest pain radiating to left arm , breathing difficulty, choking sensation in the throat, inability to open his ey
