Pulled elbow and the baby

I'd be very direct in this blog.

Aim :
To narrate a funny pulled elbow case involving a one year old baby and me.

People involved :
Parents of the baby , Me : Daactar Wajeeha , Orthopedic consultant, his registrar , my nursing team , and the baby.

Things involved :
My hands and the babys elbow.

Background :
My orthopedic knowledge is very very poor. All I did in my Ortho posting is eat and chill. My fellow male residents slogged . Hence , knowledge gained is zero. Except for long bone fractures , splinting ... I understand nothing.

Motivation :
A senior who had reduced a pulled elbow in ER . The rapid pain relief it gives.

Situation :
Baby not moving elbow, extended position . No orthopedic available. Estimated time of arrival of specialist 3 hrs. Baby inconsolable.No senior available for help for next 1-2 hours.

Pros :
Orthopedician motivates to go for it.
Restraint : Never done it before.

Options :
1. Shoo the patient out of ER.
2. Wait for help for 2-4 hours. Leaving the baby in pain.
3. Try fixing it after consulting it.First time for everything.

Scenario:
Tried finding consultants and registrars.When they gave their timelines.
I ask my friend for help. She sent me a YouTube video of how it is done. I see that , try it once. Fail. Baby was still crying.
Call up registrar for help. Remember Kareena Kapoor guiding Amir Khan to conduct a delivery 😜 .Thats a rather hue example. But orthopedics
to me is like what Gynecology is for most of the male doctors there. So , I try again , fail.
Third time , the baby started crying looking at my apron 😅. I got hold of him gave a shot, heard the sound.

Testing:
Now that I had performed the procedure , I wanted to check if I'd done it right. The best way is to check if the baby is flexing, extending, pronating and supinating his elbow. The baby was so annoyed, the moment he saw my white coat he would scream. To test I sent my nursing staff, with whom he was comfortable.

Result:
The baby was now making movement with little apprehensions though. But yes , felt good and confident.

End Note :

Emergency Physician got to know everything, otherwise you get caught up in situations like these. There is first time for everything though!

Parallel Universe : BCT Series XXX

  Two moments in the ED this week made me realize that I come from a parallel universe. One involved a teenage patient who had applied bronzer that I found while cleaning her skin for a IV cannula which left me surprised as I come from a part of the world where teenagers paint their skin with whitening agents to look more attractive. The second moment was during my teaching session on Organophosphorous poisoning, where I wondered if my accent was causing disengagement . However, I learned that it was simply a rare presentation of organophosphorous poisoning in this part of the world. It felt like I was narrating my adventures from a tropical trek! Have you ever had a moment where your cultural background clashed with your environment ?

Undermining in ED - A common sight

Hi all This us again not an original article written by me. but found this article too good, not to reshare. Ill share the link to the orginal article at the end of the post. Having seen this in and out of department since day 1 of working in corporate hospital , felt nice as I found more literature on this topic. I am yet to study in detail about it.
