Nil - Offer day

You have AMC on your last day? Thats terrible. Worst! Worst than a post duty.

Why?

Areyy! you get up . End up in a new environment all tired!... Who wants that?

Its not that big deal . Ill manage.

I said this because I didn't know what awaited me . The last AMC was pretty much good with a unit Biryani party to bid  farewell to our journey in Department of Medicine.

Hickory, dickory, dock.
The mouse ran up the clock.
The clock struck 12 ,
The mouse ran down,
Hickory, dickory, dock.

THE WAR :
Haha! Yeah it was the time to split up into shifts. Nobody was willing to shift between 4:30-9 AM . So , the ones leaving the department (Yes , you no longer work for us ! Go do the second shift 🙈 situation arose. ) All we had to do is Nod.

ZOMBIE :
The anxiety of going to the new department didn't let me sleep. So , I turned up at 4:30 AM like a zombie . The time usually is sluggish in such situations but it just flew away.

RELIEVER
In no time it was 9AM . *No sight of reliever*
Friend calls. Where are you?
*Will be there soon*
9:15AM . No reliever .

9:30AM *Forget it !lets have the breakfast in the AMC itself. *
I munch some energy bars ( Yes , I needed them to survive the day)
Friend calls up :* Leave everything ! Come and report in the department. The reliever will take forever*

9:45AM Eureka! There is the reliever!
*Books a cab * *Runs in the most filmy way ever*
- Grabs Handbag .
-Dramatically shuts the door
-Looks at the time like the flight to USA
will take off in 15 minutes.
-Finally runs with the most panicky expression ever

For the first time in forever : ENTER NILOUFER HOSPITAL
Post duty? Asked a co - intern waiting there.

Yeah.

Your apron smells like AMC ! Said a co-intern. ( Okay dude. You had a luxury to bath and report . I didn't . Buzz off! )

I hastily get in to the room to report and the lady there manually pushes me out .*TI's should come last.*

I stand there dumbstruck * Madam  sarkari naukri kaafi seriously lerahi hai!  *
With the* Beta ruk . Meri posting hone de batati hoon * wala look . I abide to her.
She allots me unit 6 . I find no familiar face .

* What a pain ! *

Can we mutually exchange our units ?

*No! *
About my unit :
The worst part about being in that Unit  was *Saturday Duty* with *bonus Sunday duty*
Who on earth works on Saturdays ? Sundays to make matters worse .

I go try my best exchanging units . Things don't work out.

Lost as usual :
While running in between two blocks I notice my face mask ! ( yes, the same one I have been wearing since 4:30AM . The same one that hung while I boarded a taxi , reported at a unit.

The Unit Meeting
The PG comes gracefully .After a brief introduction , tells us about the characteristics of the unit.

  • Forget Sundays ! I have also erased them from our calendar !( Are you kidding me ... Sunday ! A Sunday ? NOOOO! Noooo!  )

  • Try iv cannulas . Don't worry , you can poke as many times as you want. Its the practice that gets you the skills. ( NOOOOO! That baby and his mother are going to curse me throughout the day and Ill have 'n' number of new problems . I have never seen a doctor find IV cannula for an infant even in cases of acute emergency! Why am I supposed to know that? )
  • We have a duty tomorrow ! You'll learn everything! ( Okay! Sounds fun!)
Drama Queen Mode On :
The PG leaves . A case enters .
*Amma soodi *
I try placing the cannula .
*I dramatically turn around teary eyes *
 I can't poke a baby!

My co intern quite considerably explains , its okay . You are doing that so that it gets well soon . See , Ill show you. Its this simple.

I go home . Cry for an hour . Then some self consolation. For four to five weeks from now Dr. Wajeeha Musharraf will be saving lives every weekend! Sundays? Never mind! Hota hai!

With some positivity . I look ahead to the duty day.

MRCEM OSCE preparation and resources

Hello people Since I have cleared my OSCE in one go, I feel it's my duty to journal what all I have done to make it work.(humble brag). Thanks to ChatGPT for helping me pile up some humble generic sentence to make this blog wholesome.(Just kidding). The MRCEM OSCE exam is an integral part of the qualification which tests the clinical and communication skills of candidates. For Indian students pursuing this qualification, preparing for the MRCEM OSCE exam may seem daunting. However, with a few tips that helped me sail through fine. The thing that helped me the most is acclimatizing myself to the UK Clinical Setting Before appearing for the MRCEM OSCE exam, it is imperative for Indian students to understand the clinical setting in the UK. Sometimes , all they want to see is how you behave , your body language, your tone , and your organising skills. Diagnosis is not a critical thing here. You will not fail if you get a wrong diagnosis provided that you have done it systematically. So

Hello everybody  I hope everybody is safe and healthy. it's been a while that I've been receiving lot of patients in the emergency with mental illness ,thought of sharing an incident of its kind. Not sure if its the awareness of mental health these days or the pandemic, I am developing keen interest in patients with mental illness .I try to take an extra effort to make them feel comfortable and try to create an insight of what is happening and offer them options of what all can be done.  Since this it seems like a step towards patient care , I thought I'd share it here as well. This would probably not be the best approach but I welcome any type of constructive criticism , additions or subtraction to my approach because end of the day we are here for best patient care. A 50 years plus old gentleman ,presented to the emergency with the complaints of chest pain radiating to left arm , breathing difficulty, choking sensation in the throat, inability to open his ey

Professional Growth Spurts in EM

Hey all Once you're done with your residency . Things appear differently. Some people have a distant vision about such things while some realise at the moment. Nevertheless, the struggle is more or less the same. Remember going through growth spurts as a child? The too-short pant legs were embarrassing, but the achy limbs and joints made it miserable. Just like when you were a teen and all your comfortable clothes would no longer fit. These are tough adjustments “growing pains,” and they can throw your career way off its trajectory if you aren’t prepared to face them. While moving out of your parent institution is great, not everything is sunshine and roses. You can experience these growth spurts fast or slow. Some people are lucky enough to choose the pace. Some people have no choice but to give in. Confusion , Reduced morale, Unclear priorities , A decrease in productivity is very common.Sudden changes from one organization to another organization can lead to discomfo
